To save time at your appointment, please complete your new patient paperwork before you arrive. Fill out your forms online.
Please bring:
- A photo ID
- Your dental insurance card, if you have insurance
- A list of any medications you take
At your first visit, your dentist will review your health history, examine your teeth and gums, and talk with you about any treatment you may need and your options.

We accept most dental insurance plans and are in network with many providers, including Delta Dental, Select Administrative Services, Always Care, and LICOA. We also accept credit and debit cards, cash, checks, CareCredit, Compassionate Finance, and Sunbit. See our financial options.
